18 Jul 2022 – Long Term Conditions Annual Recall

PROJECT DELAY

In our last letter we explained that it was our ambition to start the annual recall initiative for Long Term Conditions (LTCs) in August. We are not , however, in a position at this time to start – and so have delayed the start date to September. Recalls have, however, been sent out to those of whom were required to come to the Practice.

Part of the delay is the focus that has been required to get the Clinical Triage initiative off the ground – and its success is helping us forge how we do the LTCs for going forward. We are also working with Microtech, the firm who provide our Clinical Document Management system, the self-check-ins, and the interactive pods; because they have developed an App which allows data to be entered from the patient directly into the Clinical System, and that has a significant effect on how our overall project goes forward.

We are working to get the initiative off the ground as soon as possible, and will keep you informed on our progress

Following on from the meeting with Cllr Reynolds, 10 Jun 22, and prior to the inaugural week for Clinical Triage, we followed up with our Practice statement which highlights the Practice, it’s challenges, analysis, planning and evaluation, and our strategic vision for the future.

Prior to the inaugural week for Clinical Triage, we were given the opportunity to discuss many of the concerns raised by Counsellor Glen Reynolds’ constituents. We presented the Practice, it’s challenges, analysis, planning and evaluation, and our strategic vision for the future.

07 Jun 2022 – Clinical Triage – A message from your GPs

CLINICAL TRIAGE – FROM MONDAY 27 JUNE 2022

Over the past 6 months we have been analysing our clinical demand in order to ensure that we are making best use of the available clinical time.  We have also listened to patient feedback where we have been less successful in our approach.  Notwithstanding that we have continued to operate seeing patients throughout the COVID pandemic hitherto, our primary focus has been with managing the daily demand of our c.12,500 patient list.  The result of this analysis is that we will soon be starting two significant initiatives; that of Clinical Triage, and Long Term Conditions. 

Clinical Triage will commence from Monday 27 June and will offer a web-based facility for patients to inform the clinicians of their primary care needs.  Hopefully removing the frustration and requirement to wait on, what appears to be, a never-ending telephone queue.  Our Clinical Administrative team will transpose the information into the patients’ respective medical journals so that the clinician is better informed prior to the consultation.  Thereafter, Clinical Triage will be clinician led, GP and/or Advanced Nurse Practitioner, in order to triage the request and effect the appropriate course of action.  This may be a telephone or video consultation; face-to-face review, or a future appointment.  Clinical Triage will be open for use from 06:00 – 11:30 Monday to Friday, and there will still be the option to telephone the surgery as normal.  In order to ensure that the information is transposed consistently, and is queued appropriately, our administrative staff are being trained to help those of whom have limited access or experience with web-based forms, by completing the same online form with them at the point of the call.

Long Term Management of Chronic Diseases will be restarting in August; and the invites for appointments will be issued from the beginning of July.  Patients will be invited in accordance with their month of birth; and there will be more information to follow.

We hope to see a significant improvement in how patients are able to communicate with the Practice, and we will be constantly assessing the initiatives in order to learn and improve. We will be providing more information on our website in due course; and we also use the medium of Social Media to signpost to the respective webpages.
