Medical Records
NHS Grampian request to make a link between your pre and post transition Medical Records.
As a trans patient, you are entitled by law to have your former gender identity kept confidential. However, if you have a pre-existing health condition when you transition, for example diabetes or a cardiac condition, it will still be present after transition. It would greatly assist doctors treating you in future to know your full medical history. This information would be restricted access only.
NHS Grampian would like your permission to make a link between your old and new Medical Records.
