Please use the attached link to our Online Form for you to keep us informed of your epilepsy status. We will send out an annual request for this information but you don’t have to wait for this to happen. Use the form to let us know as soon as your condition changes. We will enter the data into your clinical journal; and the clinicians will be notified if you have informed us of any significant concerns.
