Clinical Triage Request Form

Clinical Triage (CT) is where a clinician looks at all of the received request forms and prioritises them in order for the patients to be contacted by an appropriate clinician. The elements of CT are Telephone, and Urgent on the Day for those of whom experience urgent symptoms post the CT window. Every request is subject to CT.

Urgent on the Day Requests (From 14:00)

When you contact us about a condition that you consider as requiring an urgent on the day consultation you will be asked a series of questions that the clinician will base their triage upon. The information given by you will be recorded on a Practice form and included in your medical journal. The clinician will determine the outcome of your request based upon this information and this will be recorded in your medical journal. The outcome of the request could be:

  • An initial telephone consultation
  • A Face to Face consultation
  • An SMS response with advice:
    • Signposting to an appropriate facility
    • Complete a Clinical Triage form the following day
    • Urgency dependent – contact 999
    • Time dependent – contact 111 after 18:00
